To apply for membership please complete this form and present it with a check for $10.00 to the Forest Advisory Council.  We meet the 2nd Monday of each month at 7 pm. at the community center. You will need Adobe Reader to be able to complete the form.  Please be sure to print the form and not save it, otherwise your info will be available to the next user.
